CASE MANAGEMENT

ACRS is committed to helping Asian American and Pacific Islander elders and adults with disabilities lead meaningful and healthy lives. Our bilingual case managers communicate with clients in their preferred languages, conduct in-home assessments, and develop personalized care plans to improve the lives of clients and help them stay in their own homes for as long as possible.

Our staff also coordinates with the Department of Social and Health Services (DSHS) to care for those recently released from hospitals, nursing homes or other long-term care facilities.
