WELLNESS AND WISDOM PROGRAM
Improving access to services for older adults with substance use disorder (SUD) involves active outreach, education, in-home services and other anti-stigma activities. Our Wellness and Wisdom Program activities include:
- Screening for substance use and/or mental health disorders.
- Referrals for drug and alcohol assessment and treatment.
- Providing drug and alcohol treatment, and coordinating care with other care management teams.
- Supporting partner agencies with on-site training and improving their ability to identify health-related needs for services.
- Outreach to clients who are resistant to treatment.
- Improving referral and service delivery among local assessment and treatment agencies.
- Coordinating screenings with other treatment providers.
- Distributing information and providing education about the program to other treatment providers to ensure coordination of care.
