HEALTH HOME & CARE TRANSITIONS

The Health Home program provides long-term care coordination for Medicaid-eligible individuals by developing person-centered health action plans. It aims to empower participants to better manage their chronic conditions and maintain their overall well-being. The program connects individuals to both medical and social services to ensure comprehensive support.

Meanwhile, the Care Transitions program offers short-term support to adults returning to their communities after a hospital stay. The program focuses on stabilizing participants’ health and ensuring they have the necessary resources to remain healthy and independent at home. Both programs emphasize the importance of continuity of care and promoting well-being within community settings.
